Transfer Your Home Care Package to ComLink

The Australian Government is increasing opportunities for individual choice. You now have the right to make more decisions about the care services you receive. The most significant change to date is, you now hold the Home Care Package, instead of your current service provider.
What does this mean for me?
If you are currently receiving care services through a Home Care Package, you now have the opportunity to change who provides the care services to you, because your Home Care Package follows you.
Why would I change service providers?
There are many reasons a person might change service providers, including:
- You move to another area
- You are dissatisfied with the care services you currently receive
- The service provider is not meeting your needs or goals
- The service provider fees are too high
- Or, no reason at all, it’s simply a choice you wish to make!

How do I transfer my Package to ComLink?
Changing your Home Care Package to ComLink is easy! In fact, ComLink will do most of the work for you, to ensure it is an easy transition with minimal disruption to you and your care needs.

Speak with ComLink to ensure your needs and goals will be met and advise us you would like to transfer your Home Care Package to ComLink. Simply call ComLink on 1300 761 011.

You refer to your Home Care Agreement with your current provider to determine if you are required to give a minimum notice period to leave their services and pay an exit fee. ComLink will cover your exit fees if you decide to switch!*

Notify My Aged Care and your current service provider of your intention to move your Package. ComLink can help you write the letter if you require and will assist you with the process to finalise the switch.
